If the island where you will be travelling is geographically not attached to Europe, unless it is included in the list of European countries of the policy wording, you will need Worldwide cover. This applies even if the island is an oversea territory of a European country.
For instance: If you will be renting a car in Martinique or Guadeloupe, you would need worldwide cover. If you will be renting a car in the Canary Islands however, the European policy is sufficient as these islands are listed within the definition of Europe on the policy.
